Narrative
Approximately 2 barrels of crude oil were released from low-pressure surge relief station piping at corpus christi west. The date and time of the release (part a q4) is indicative of the earliest possible time the release could have started as it was the last time the incident location had been visited and confirmed to be no release of crude oil. The cause of the release is attributed to internal corrosion and the investigation is ongoing into the sub-cause. To prevent future releases, the flange was removed from service and replaced. A supplemental report will be submitted pending further investigation into the sub-cause and additional preventative measures. Supplemental: the cause of the incident is attributed to microbiologically induced corrosion based on field examination. Additionally, a third-party analysis of the continuity at the facility was performed, which found that anodic/cathodic interference on one side of the failed flange increased the rate of corrosion. In addition to the previously mentioned preventative measures, isolating gaskets were removed to ensure continuity throughout the facility.
