Narrative
Approximately 3.8 barrels of crude oil were released from the pig launcher for barstow to wink 16inch. The date and time of the release (part a q4) is indicative of the earliest possible time the release could have started based on the last time the station had been visited and confirmed to be no release of crude oil and local operations knowledge of the release rate and size of the release. The cause of the release is attributed to incorrect operation as a result of an incorrectly sized o-ring installed on the pig trap door. To prevent future incidents, the employee was re-trained, plains will further investigate updates necessary to applicable procedures, and the o-ring that failed was replaced with an appropriately sized o-ring. Supplemental: updates applied to the dates/times provided in part a.
