Narrative
On june 14, 2023 at 10:43am, enterprise was notified by a 3rd party of a ice ball on a valve. A pipeline technician was dispatched and arrived on site to confirm the ice ball was on an enterprise pipeline valve at 11:30. Enterprise dispatched a team to contain the leak area and investigate the source of the leak on the valve. At 1:45 pm enterprise determined that based on the leak rate observed, the leak volume would exceed the 5 gallon threshold and the rrc was notified at 2:00 pm, and assigned incident number 2846. At 6:43 pm, enterprise shut down the pipeline in order to remove and replace the leaking valve. The valve was sent in for examination and it was determined the leak was due to a failed o-ring that was cracked.
