On june 12, 2023, a colonial employee was conducting normal station maintenance activities at the boligee delivery facility in boligee, alabama. The employee observed an area of stained gravel. Upon further investigation, the employee identified a cap missing from a tubing connection on the pressure switch downstream of the line 449-2 meter run control valve, which was determined to be the source of the release. An investigation is on-going. Update: an accident investigation determined that the cause of the release was equipment failure. The failure occurred due to excessive vibration experienced on the line during deliveries. These vibrations caused the cap to loosen over time, and ultimately separating completely. An action plan has been developed to address the excessive vibrations experienced on the line at boligee delivery during normal operations.
