On june 12, 2023, at approximately 0900 central time, the station operator was performing the weekly station checks when he noticed crude oil on the pump skid and around the pump base. He immediately contacted supervision and a stop help start was initiated. Upon further investigation it was discovered that a section of stainless-steel tubing for the bearing cooling system had worn a small hole from the inside due to severe turbulence in the flow through the tube due to a ball valve that was not fully open. The section of tubing was replaced in addition to the ball valve. 08/22/2023 supplemental final made change from "jetting" to flow turbulence and added the partially closed ball valve to the narrative as a cause of the turbulent flow.
