On june 7th at appx. 9:15 am cst, a terminal technician was performing their daily rounds at the el dorado west tank farm when they noticed a leak occurring from the mixer of a breakout tank (tk 4154). The technician immediately shut off the tank mixer and then notified their supervisor and the hep control center. The technician then began placing oil hogs (absorbent pads) down. In total appx. 25 gallons of crude was released. Onsite contractors were able to assist with clean-up efforts. In all, affected soil for the appx. 25 gallons released was able to be dug up and placed in drums for permanent disposal. The nrc was contacted at appx. 9:42 am cst regarding this release. To note, this line is generally receiving off a regulated, mainline (the osage pipeline via manifold). However, on the day of the release, valves were closed and the tank was not receiving off the mainline. The apparent cause of this release was due to a seal failure on the mixer. Specifically bearings had gotten loose causing the shaft of the mixer to move back and forth allowing for crude to leak from the seal.
