Narrative
Approximately 49.83 barrels of crude oil were released from station piping at midland pmt. The date and time of the release (part a q4) is indicative of the earliest possible time the release could have started based on the last time the incident location had been visited and confirmed to be no release of crude oil. Prior to the release, the segment of pipe that failed had been isolated from service pending removal, however, a valve failure allowed for oil to re-enter the line. The cause of the release is attributed to internal corrosion as a result of low-flow/no-flow operating conditions. The internal corrosion determination is based on visual examination of the release point and review of the operational history of the line. To prevent future releases, a clamp was placed, and the line was drained and air-gapped pending removal.
