On thursday, june 8th, 2023, at approximately 20:00 cst, an operator discovered freestanding diesel in the shipper valve check-valve vault. There were three check valves in this vault (one gasoline line and two distillate lines). All three lines were inactive and were isolated at the time of discovery. All appropriate notifications were made. Personnel were mobilized and arrived at approximately 22:25 cst and began spill recovery and dewatering activities so the source could be investigate/identified. On june 9th, 2023, at 15:30 cst, a 16-inch valve flange on a nearby purge line, outside of the vault was found to be the source of the release. As a result of the release 138 gallons of diesel was released and recovered. All released product was contained on cpc property. Repairs were completed and the line returned to normal operational status on june 10th, 2023, at 15:38 cst. On june 9, 2023, at approximately 14:10 cst, it was discovered response actions may meet or exceed $50,000, thus, a nrc notification was made (see report no. 1369619). The nrc 48-hour update information provided corrections to the following information: date of release discovery to june 8th; the $50,000 nrc notification threshold not being met; and to the reduced release volume (see report no. 1369760). September 9, 2023, a supplemental final report was submitted to PHMSA with the following revisions: subpart a(14) - onshore terminal or tank farm equipment and piping.� subpart c(3) - flange assembly, including gaskets.
