On june 5, 2023, at approximately 15:00 cst, a technician performing a station check at epes noticed stained gravel and a sheen on water in the check valve vault of unit #3 on line 1.� appropriate notifications were made.� the control center was notified, and the line was isolated at approximately 16:22 cst.��� additional personnel were mobilized and arrived at approximately 19:00 cst, to further investigate, repair and perform spill recovery.�� on june 7, 2023, at approximately 14:33 cst, the source of the release was found to be originating from the inboard seal of line 1, unit 3.� the alarm associated to this equipment did not activate. As a result of the release 207 gallons of product was released. �to date, 85 gallons has been recovered. The pump seal was removed and is being further investigated by the colonial engineering department and the manufacturer in an effort to determine the cause of failure and the possible need for additional actions.� the unit will remain isolated pending replacement parts and proper function of the alarm.� � on june 5, 2023, at approximately 16:21 cst, it was discovered the release would meet or exceed $50,000, thus, a nrc notification was made (nrc no. 1369215).� on june 7, 2023, at approximately 16:38 cst, the 48 hour nrc notification was made (nrc no. 1369410), this notification provided update to the volume released. Follow-up: colonial engineering and the manufacturer found the cause of failure to be metal fatigue; the component springs failed over extended use of time. Additionally, the final volume recovered was 130 gallons. The unit has returned to normal operations.
