Narrative
On 6/7/23 at approximately 07:25, an operator discovered a release of approximately 8 barrels of crude oil from the pig trap tubing onto the ground surrounding the st. Martinville pump station scraper trap. The trap was isolated and cleanup contractors were dispatched to remove free product and contaminated soil and rocks. On 6/12/23, it was determined that total costs would exceed $50k due to environmental remediation costs exceeding those which were initially anticipated for a release of this size. Immediately following determination that total costs would exceed $50k the nrc and PHMSA aid were notified. Operations assessed the scraper trap and the investigation determined the release was caused by a tubing failure due to improper tightening of a swagelok fitting when installed during construction. Due to the fitting not being properly tightened according to the manufacturers procedure it prevented the ferrule from properly crimping the stainless steel tubing resulting in the tubing coming out of the fitting and causing the release. Repaired the failure by reinstalling the tubing to manufacturers specification. To prevent reoccurrence, operations assessed all other tubing fittings on the system using swagelok procedure and gauge to check fittings for proper tightness.
