Narrative
On may 24 at appx. 11:30 p.M. Central, the hep control room received notification of inboard and outboard seal failures from a pump (unit 3) at station located in orla, tx. The line feeding this pump was shut down, the area supervisor contacted and local operating personnel dispatched to further investigate the alarm. Local operating personnel arrived onsite at appx. 1:30 a.M. Central. At appx. 2:39 a.M. Central, local operating identified that a leak of appx. 1 bbl of gasoline had been released as a result. To note, scada-based information assisted in detection of a failure but did not provide definitive data of an actual release. In addition, cpm is available but solely on the regulated, mainline which this pump (point of failure) feeds. Direct observation was needed to both confirm a leak occurred as well as identify initial, released volume. Because of this release, agency notifications were made to the national response center (nrc). After completing excavation of the impacted soil, both the released and recovered amount were confirmed to be 1.8 bbls. Also to note, part e of this report indicates an estimated pressure at the point of release as 1,100 psi. However, due to various mechanical components and 'stages' the commodity cycles as it flows through the pump and, that this was a failure of both the inboard and outboard seals, the actual pressure would be somewhere between suction psi and half of the discharge psi. In investigating this release, the apparent cause of the inboard and outboard seal failures was due to heavy build-up of sediment (trash and other materials) coming down the line and into the pump. This buildup likely occurred from previous, turnaround activities at hf sinclair's navajo refinery which, feeds the mainline that goes into this pump.
