Narrative
During routine operator rounds, company personnel discovered a release from a pump seal on a regulated pump. The pump was not running at the time of the discovery and therefore the release was contained within the pump basin and surrounding area. All product was contained on operator-controlled property. After discovery, the operator who discovered the release notified all necessary company personnel and supervision. On-site contractors and company personnel immediately initiated clean-up/remediation activities. After a failure analysis was performed it was determined that the failure was caused by abnormal wear due to frequent startup and shut down cycles. This is a multi-use pump that is not dedicated to DOT operations.
