Narrative
On 5/19/20 at approximately 11:30, the associated prv piping developed a crack above the weld coming off of the 30" ship loading line. Operations was notified to shut down the loading of the ship in order to stop the leak and make the necessary repairs. It was determined that approximately 2 bbls of crude oil was released as a result of this broken prv line. Vacuum trucks responded to the site along with personnel for cleanup. Once the line was slacked, loto was completed and a plug was installed in the piping. A welding model was designed and a 4" flange was welded onto the pipe to facilitate a new prv and associated prv piping. The ship loading was restarted after the piping was completed and loto removed. All prv settings and designs within each manifold are being assessed by engineers to prevent reoccurrence.
