On may 22, 2023, at 11:14, the findlay operations center received a call that crude oil had been found after a relief line flush procedure. The procedure called for the owensboro to catlettsburg system to be shut down and the flush was completed with the use of an onsite booster pump from tankage. After the flush, crude oil was spotted just over the location of the relief line. Upon excavation, using company equipment and personnel, a pin hole was discovered in the relief line, the line was repaired using a plidco and cleared for restart at 16:03. Supplemental updates part a - osro activation and arrival time, times of confirmed discovery and failure identification part c - item involved in incident updated to pipe.
