On may 13, 2023 at 10:50am (mdt) oneok pipeline control received a call from a 3rd party operator who reported a potential ice ball at oneok linam ranch meter skid. A oneok operator was dispatched and arrived on site to discover a release on the oneok launcher door. The launcher was isolated and evacuated of product. Upon investigation, the operator found the launcher door o-ring had failed. The o-ring was replaced, product reintroduced to the launcher with the operator remaining on site to ensure normal operations. Oneok reviewed pressure transmitter data and determined the o-ring failure likely started at approximately 5:50 pm on may 12th. Update: oneok submitted the failed o-ring to a third party for analysis. The third party analysis determined the o-ring failed due to a decompression event and did not find any indication of o-ring material degradation. The o-ring material itself, as compared to an exemplar, appeared to have maintained elasticity, had appropriate hardness, appropriate chemical analysis, and a 1.1 percent deviation in the o-ring circumference measurement. The lab was not able to determine the cause of the decompression event. Oneok verified the o-ring was installed per oneok's procedure.
