Narrative
Permian anchors was drilling new anchors at a location approximately 4,600 feet from the location indicated on the one call ticket placed. Due to this misinformation, wwm operating, llc was not listed on the operators to clear the one call ticket. The excavator hit the pipeline and notified the operator he was working for. The operator called the emergency number on the wwm operating, llc pipeline markers and notified gas control. Wwm operating, llc determined it had punctured the pipeline and was leaking gas. Upstream and downstream mlvs were closed to isolate the damaged section of pipe. Damaged section of pipe was depressurized and repaired before returning to service. Permian anchor was contracted by chevron. A representative of chevron informed wwm control room of line strike on 11/16/2020 at 13:52 central time.
