On 10/04/2023 at approximately 443pm, bt-1-an, a natural gas transmission pipeline, ruptured and ignited at jessieville junction. -443pm: system control was notified by a caller from the general public at 443pm -445pm: the lead gas controller began notifying ops personnel at 445pm of a possible failure at jessieville junction. -447pm: the lead gas controller received a rate of change alarm indicating a rapid loss of line pressure. -529pm: ops personnel arrived on site and confirmed that a pipeline had failed at jessieville junction. -531pm: ops personnel isolated the downstream valve 16" bt-1-as valve at hwy 88. -544pm: nrc notification was made (nrc# 1380909); 48hr nrc update (nrc# 1381352) -615pm: determination was made that above ground piping for bt-1-an 24", bt-1 16", bt-1-as 16", and bt-1 30" were affected at jessieville junction. -630pm: ops personnel isolated the upstream valves for bt-1 16" and bt-1-an 24" at dry fork mlv ops personnel manned the valves at dry fork and hwy 88 while additional technicians stayed onsite at jessieville junction throughout the night to monitor the flame as it burned out. Three people were evacuated from a home approximately 645' from the failure site on 10/04/2023; they were evacuated at the time of incident and able to return to the home at approximately 730pm that evening. 10/05/2023 -744am: the fire at the failure site was confirmed to be completely extinguished. -800am: ops personnel initiated the operation of air movers -1255pm: failure site was cleared for access and personnel from aid and the sw region were able to begin their investigation 10/07/2023 -the suspected origin failure piece (bt-1-an) was loaded on a truck to be shipped to adv integrity lab in magnolia, tx -contract crew began excavating bt-1-an, bt-1 16", bt-1 30", and bt-1-as 16" to the appropriate location on each segment to cut and cap. The metallurgical testing results for the origin piece of pipe are pending and a supplemental report will be filed. On 10/13/2023, energy transfer received a corrective action order (4-2023-007 cao egt) defining the requirements to return bt-1-an, bt-1 16", bt-1 30", and bt-1-as to service from dry fork mlv to hwy 88 gate valve. The segments remain isolated and out of service at the of this filing. Customer impact -there are 12 total taps and 1 town border out of service due to the isolated segment from dry fork mlv to hwy 88 gate valve being out of service. -8 domestic style taps and 4 rural extension u-shape taps -hot springs #2 town border -the local distribution company in the area has 244 customers affected with the isolated segments from dry fork mlv to hwy 88 gate valve out of service. *supplemental report updates* part c - additional facility information was updated: -3c: smys is 60,000 -3f: pipe manufacturer is a.O. Smith/ US steel -c6: type of release is longitudinal and approx. Size of release is 30.0in x 538in part g - apparent cause was updated: -g5 - "material failure of pipe or weld" was selected as the apparent cause. -determined by metallurgical analysis. -environmental cracking-related "stress corrosion cracking". -post construction pt pressure was added part j - integrity inspections -updated with most recent and previous tool run information. Updated supplemental and final report with date and time that flow was established on bt-1-an. The pipeline was officially restarted on 3-28-2024 at 19:20.
