Narrative
Crew was on site investigating a grade 2 leak. While conducting the leak analysis and trying to pinpoint the leak's location, a damage occurred by probing through the 3" plastic main creating a grade 1 leak. The crew began to excavate to make repairs on the grade 1 leak. At this time, the crew did not follow established procedures while attempting to make an unauthorized repair by installing a mechanical "tee" on an uncontrolled release of gas on the main. The gas ignited causing burn injuries to 2 employees. One of the employee's injuries required overnight hospitalization. The grade 1 leak was repaired the same day. The grade 2 leak was repaired at a later date.
