Narrative
On 10-13-2023, a third-party contractor damaged an 8-inch steel gas main at 4200 valley blvd, walnut, ca while grading. Socalgas crew responded and controlled the release of gas. There were no injuries or further damage resulting from this incident. Investigation of the incident showed there was a valid USA ticket. However, the contractor's excavation practices were inadequate as it failed to pot-hole the 8-inch main. This incident was reported to DOT and cpuc due to gas release exceeding 3,000,000 cubic feet. Note: on 11-16-2023 a change was made to part a 4 to reflect the request from PHMSA to change the time from time incident became reportable to the time the company was notified. Per PHMSA note "earliest time would be the time the 3rd party noticed the leak. A4 is not to be confused with the time of confirmed discovery."
