Narrative
On september 7, 2023, an explosion occurred at 14:18 at 4100 ridgeley ave ne in albuquerque, nm 87108. At 14:37, nmgc was notified by albuquerque fire and rescue of a house explosion. Nmgc arrived at the property at 14:52. Gas was escaping from the riser when the nmgc technician arrived. Gas was under control at 15:33 at the riser. Nmgc began probing for gas at 15:33. The nmgc responding crew discovered gas concentration readings of 100% gas in the street directly in front of 4100 ridgeley ave ne. Nmgc exposed the 2-inch pe pipe, where the section of main was offset with a dogleg that was installed in 2017. A 2x3-inch leak clamp was installed as a temporary repair. The section of main was squeezed off at 21:30 and capped as a permanent repair. There were two residents of 4100 ridgeley avenue who were treated for non-life threatening injuries and released. At 17:15, gas odorant testing with a heath odorometer was conducted revealing sufficient odorant at 0.50% gas-in-air (12.5% lel). Additional gas odorant testing with a heath odorometer was performed showing appropriate odorant levels. Laboratory tests were conducted on october 25, 2023 which revealed a lack of complete fusion on butt weld. Regarding questions part a #7 and part a #21c, the form does not allow an entry of "unknown" because a calculation cannot be performed due to limited or unknown information needed to perform the calculation.
