Hartco cable, inc. (third party contractor) bored into a 16-inch natural gas steel main while installing cable tv at 1400 nw 86th street in clive, iowa on august 2, 2023. The boring head punctured a four-inch by two-inch hole in the lower third of the main that was at a six-foot depth. No services were interrupted and there was no ignition or injuries. Due to the size and depth of the main, it took a significant amount of time to repair the damage and 6.771 mcf of natural gas was released to atmosphere. The nrc was initially notified (report 1375040) on august 2 and a 48-hour follow-up report was submitted (report 1375241).
