Following a service valve replacement, environmental conditions allowed for residual gas to be ignited and resulted in burns on two employees. Note - for question a4 the district used 8:30 am at the direction of PHMSA. This was not the time that the incident became reportable. That time was when the employee was hospitalized which occured at 13:30. Note - for questions a7 & a21 the district used 0.1 at the direction of PHMSA. All gas released prior to this incident was intentional.
