Narrative
A third-party fencing excavator damaged a one-inch plastic service line with an auger attached to a skid steer. After striking the service line the skid steer operator backed up slightly and a coworker began to remove soil from the hole when gas ignited burning the contractors face and hands. In addition, the fire damaged the skid-steer. The third-party fencing excavator called in a one call ticket to a building one street over. The damage occured at a location without a one call ticket. The odorant reading at the time of the incident was .42 and due to a system issue, I was unable to document the reading in section e6. Please consider this report both and initial and final report. No further reports will be filed.
