Narrative
Peoples gas received a leak report on 1/12/23, responded to the scene and began an investigation. A determination of the source and location was identified, and the scene was made safe with first responder support. Based on the location of the identified leak, depth of pipeline and potential customer loss, the corrective action plan to make the scene safe was to install a temporary by-pass to remediate the identified leak. The damaged pipe was isolated, and the leakage stopped. A new pretested section of pipe was installed for permanent repair, and the temporary bypass was capped and taken out of service. During the assessment, a previous unreported longitudinal gouge (excavation damage) was identified on the 8" pipeline, that over time resulted in a leak. Leak survey performed on 9/27/2022 did not identify a leak at this location. Part g, #9 is "no" due to limitations of the form, in fact is unknown whether a locate ticket was called or not at the time of the excavation/damage.
