Narrative
At 03:25 on november 27th, 2022 magellan operation personnel were pulling a sample from a yz analyzer when product was discovered on the ground. The employee determined the drain valve on the sampler was left open after maintenance earlier that morning. The drain valve was closed and cleanup activities were initiated. Further investigation determined the open drain valve allowed the sample recovery system to overfill and overflow. To prevent a recurrence, site personnel developed an instruction manual for technicians to use during maintenance on the sampler and the current drain valve will be replaced with a dead man's valve. Additionally, the procedure 9.01-adm-046 prepare equipment for maintenance, repair, removal or replacement, which covers sampler maintenance, will be reviewed with the technician with emphasis on valve alignment.
