On november 29, 2022, at approximately 11:00 am, ironwood local field operations was notified by a third-party operator that there was a leaking fitting on the pig receiver for the gray oak lateral. Ironwood operators were dispatched to the location to investigate the leak report and found the pressure indicator on the pig receiver leaking. The fitting was tightened, and the leak stopped. The operator then isolated the pig receiver from the pipeline and relieved the pressure on the receiver to allow them to remove the fitting and investigate the cause, it was determined that the o-ring failed due to ambient temperature changes. The o-ring was replaced, no repairs were made to the pipeline. The release volume was calculated to be 1.6 barrels. There were no injuries, fatalities, unintentional fires/explosions, evacuations, or road closures as a result of the leak. To prevent re-occurrence of the same type of accident ensured the correct type of o-rings are being used and inspected other sites with the same type of configuration.
