Narrative
While inspecting valve site contractor identified that a small weep was coming from the valve stem. Contractor contacted p66 employee and upon arrival and unsuccessful tightening of packing the control center was notified to shut down the pipeline and isolated. Repairs were made to packing and weep was stopped. Line started back up that same day. Valve has since been replaced. 1/25/2023--per the accident team recommendations were to update g-6 to reflect the block valve and the sub-cause to reflect the stem packing was leaking.
