Narrative
At approximately 1248 cst on 11/21/2023, an operator reported a leak on the eastbound pump (obp 43002) at pit 4 (kinder morgan pasadena terminal). Upon further investigation, a pinhole was found on the pump head. This resulted in a leak of 3 gallons of gasoline to leak into concrete containment. The product was immediately evacuated by vacuum truck, the pump was stripped to stop the leak, and a service request was entered (#94022). Management, safety, maintenance and all proper notifications were made. A spare pump was available for installation. The leaking pump was removed from service and a preliminary field investigation was performed. Repairs conducted in 2021 were the cause of this leak. Liquid penetrant examination confirmed a weld defect (pin hole) on the repair sleeve was the cause of the leak. Original report 20220289 - 37500 submitted on 12/21/2022, under section g6(6) was marked "yes" for manufacturing defect. After gathering additional information, a supplemental report 20220289 - 37734 was submitted on 2/21/2023. The supplemental report was edited and the "yes" was unchecked on for manufacturing defect under the aforementioned section.
