Narrative
On november 19, 2022, at 2020 cst, a leak occurred at the collins tank farm in collins, mississippi. Immediate actions were taken to isolate and stop the release. All product was fully contained inside the collins facility. The national response center was contacted on 11/20/22 at 1447 after it was determined the cost of the cleanup, etc., was going to exceed $50k. 3/1/2023: investigation showed that the vertical seal ram shaft (hydraulic cylinder valve) failed (sheared) and retracted completely from the packing gland located on the top blind flange of the prover, exposing a 2.5" diameter opening for product to be released.
