Narrative
On 11/21/2022 at 10:21, operations personnel discovered a crude oil leak at the johnsons corner pump station building on the unit 3 suction piping. Station was shutdown to identify where leak was originating. Once found, unit 3 was isolated and locked out, and units 1 and 2 were restarted. Approximately 92 gallons of crude oil was released to a concrete floor inside an engine room. The released material was collected using a vacuum truck. Any residual oil left on the concrete surface was removed with absorbent pads. A visual examination determined the piping cracked just above the weld where the 2-inch drain line ties into the suction piping at the 6 oclock position. Operations performed segmented cutout and installed a 24-inch pup of pre-tested x-70 pipe. Due to initial assessment, operations went ahead and performed, like/kind, replacement of the other two systems due to susceptibility of similar events. The failed section of pipe was sent out for metallurgical analysis to determine failure mechanism. Upon receipt of laboratory report a supplemental 7000.1 report will be filed. Supplemental the results of the analysis indicate that the pipe leaked as a result of fatigue crack initiation and growth at the fillet weld of the reinforcement saddle. The fatigue crack initiated at the pipe side weld toe of the fillet weld that joined the reinforcement saddle to the suction pipe. The crack propagated through-wall, producing the leak. There was no evidence that metallurgical defects played a role in the failure. Other pump stations on the pipeline system were inspected for like and kind drain line installations and none were found.
