Narrative
Phillips 66 employee arrived on site to buffalo station along the glacier pipeline system and discovered crude oil on the ground within the manifold area for the station. Employee notified the p66 control center in bartlesville ok. And had the glacier pipeline system shutdown and isolated with closure of valves into and out of the station. Employee made internal notifications, this initiated notifications to external stakeholders. Employee initiated phillips 66 response plan and cleanup process. Supplemental report - 2-13-23 - (updated current costs of spend and failure of prv) awaiting final cleanup and repair costs for final report. Pressure relief valve (prv) failure- the valve that failed is a balanced bellows seal style prv. This style prv requires a vent on the bonnet to prevent atmospheric pressure to build up inside the bonnet and bellows for the prv's proper functionality. Moisture from the environment (rain/snow) entered into the bonnet & bellows seal, the moisture froze causing the bellows seal to expand and distort inside the prv creating the failure of the bellows seal. Upon the bellows seal failing in the valve, crude oil entered the bonnet of the valve and then exited through the open bonnet vent to atmosphere. The prv is being replaced with a conventional prv valve (non bellows style requiring a vent) via the phillips 66 moc process. 8-10-2023 final costs for release updated on part d- final report update final 8/28/2023
