Narrative
On november 10, 2022 at 05:46, magellan operations personnel discovered product releasing from a tank pump relief line where a 1/4" threaded connection had broken off. The leak source was isolated and cleanup activities were initiated. It was determined that the analog input module had failed which gave the transmitter a false, zero pressure reading causing the vfd to ramp up and operate the pump at the maximum rate creating excessive vibration and shearing of the 1/4" connection threads. The threaded connection was replaced and a new analog input module was installed.
