Narrative
Medallion maintenance personnel arrived at howard booster station on november 3, 2022 at approximately 09:30 and found a leak occurring from pump p-18600. He immediately called the control center to isolate the pump and stop the release. Medallion response team arrived and began containing and recovering the free-standing crude oil. The investigation found a braided-stainless steel hose (swagelok) on the seal flush tubing line of pump p-18600 developed a hole, releasing approximately 155 bbl of crude oil. 135 bbl were recovered with vac trucks and returned the system via howard station. Approximately 20 bbls remained in the soil. Remediation of the affected soil has begun and expected to be complete in first quarter 2023. The hose was found in contact with the frame(floor) of the pump skid. The wire mesh deteriorated due to abrasion caused by vibration and contact with the frame. The hose was replaced and the new hose was installed in a manner that prevented the hose from contacting the frame. To prevent recurrence medallion installed a new hose in a manner preventing abrasive wear. Awareness training was provided to operations personnel for recognition and repair/reporting of the potential for abrasive wear to hoses. Medallion also inspected then modified and/or repaired, as needed, all seal flush tubing lines across pipeline system by 11/21/22. Soil remediation was completed 1/20/2022. Additional notification detail: railroad commission of texas oversight and safety pipeline safety inspector michael valdez received initial telephonic report of incident 2731 at approximately 11:15 a.M. On november 3, 2022. Inspector valdez arrived on-site at approximately2:45 p.M.On november 3, 2022. The national response center was notified at approximately 10:13 a.M. On december 1, 2022, due to the release exceeding costs of $50,000.00. There were no fatalities, injuries, fire, nor explosion associated with the reportable release. There were no evacuations, no traffic re-routed, or media present as a result

Narrative
Medallion maintenance personnel arrived at howard booster station on november 3, 2022 at approximately 09:30 and found a leak occurring from pump p-18600. He immediately called the control center to isolate the pump and stop the release. Medallion response team arrived and began containing and recovering the free-standing crude oil. The investigation found a braided-stainless steel hose (swagelok) on the seal flush tubing line of pump p-18600 developed a hole, releasing approximately 155 bbl of crude oil. 135 bbl were recovered with vac trucks and returned the system via howard station. Approximately 20 bbls remained in the soil. Remediation of the affected soil has begun and expected to be complete in first quarter 2023.
