Narrative
On 10/17/2022, at 0100 (central), kinder morgan liquids terminals operations personnel at galena park noticed a hydrocarbon alarm. The location of the alarm was at tank 80-21/22 pump manifold. Operations personnel were dispatched to investigate. When they arrived at the location of the alarm, they found a sample port (threaded fitting), near a pressure relief system had broken off and product was leaking into the concrete containment. An estimated 10 gallons of gasoline blend stock leaked into the concrete containment. Only the affected pump was shut down and the affected piping was isolated. The failed component was removed and sent out for metallurgical analysis. The metallurgical analysis determined that the threaded nipple (bleeder on thermal relief piping) failed due to high-cycle fatigue initiating at the thread root of the nipple connection. New piping was installed, and the pump was returned back into service the next day. 2/26/2024 - gps coordinates were updated.
