Narrative
Approximately 125.7 barrels of crude oil were released from sump 1480 at state line station following a storm which caused a power outage. The date and time of release is indicative of the earliest possible time the release could have started based on control center data. The cause of the release is attributed to equipment failure of a check valve. To prevent future incidents, plains replaced the check valve that failed. Additionally, incorrect operation was identified as a contributing factor as the third-party control center failed to follow procedures and notify plains employees of a control center alarm. A supplemental report will be submitted pending further investigation. Supplemental narrative (1): the cause of the check valve failure was likely due to debris being caught in the valve, or the flap within the valve temporarily hanging open. Upon further investigation of the third-party control center and local actions taken, it was determined that the third-party control center did not contribute to the incident and notified the plains operator as required, but the operator failed to properly investigate and prevent the release. The operator who had been notified is no longer employed with plains. Supplemental narrative (2): following the initial investigation, there were no issues found with the manufacturing of the valve; removed "manufacturing defect" from part k. While lightning from a storm contributed to a power outage at the station necessitating an on-site response, it was determined to not have contributed to the release as the operator failed to properly investigate and prevent the sump from overfilling [see supplemental narrative (2)]; removed "lightning" from part k.
