6.5 barrels of product was released onto the facility piping and gravel pad at the david city pump station through a pin that had completely backed out of the bleed plug on the upstream piping of a thermal relief pressure safety valve. Investigation into what caused the pin to back out is ongoing. There was no impact to water and the pump station was immediately shut down upon confirmed discovery of the release. 20230207 update: the root cause analysis concluded that the station discharge psv relieved below the setpoint while the pump station was still in operation. Product began to relieve to the sump via the relief line per design. This further caused the adjacent 1-inch psv branch piping to vibrate, forcing the bleed plug to back out of the needle valve.
