On september 25, 2022, at approximately 1413 hrs. Pdt, a torrance valley pipeline (tvp) operator, conducting facility rounds, arrived at lebec station and discovered crude oil product on facility grounds originating from a positive displacement (pd) injection pump which is used to re-inject crude oil from a breakout tank to the suction side of the station's mainline pumps. The operator immediately contacted the pipeline control center to shut down the facility operations; the mainline remained in operation. The pump was promptly isolated, and the source of the release was controlled. Cleanup and recovery crews were dispatched promptly to commence response efforts. Initial release estimate exceeded 5 barrels of crude oil to secondary containment and soil. Internal and external notifications were promptly made. On september 25, 2022, at 1420 hrs. Pdt, cal oes was notified (control number #22-5632). At approximately 1432 hrs. Pdt, nrc was notified (report #1348239), and within forty-eight hours, on september 26, 2022, a supplemental nrc report was made (report #1348346). The 48-hour report included an update to approximately 127 barrels released. The source of the release was determined to be a failed seal packing from the discharge side of injection pump #2. The existing packing was removed, and the seal rope packing was replaced. Once repaired, the injection pump resumed operation on september 26, 2022. As a mitigative action, tvp is implementing sump-related leak detection capability at this location. Additional proposed mitigative actions include sourcing packing gland alternatives.
