Narrative
On 9/24/2022 at 09:25, a denbury contractor arrived at mlv-02, observed a CO2 leak from the receiver facility and immediately notified denbury personnel of the leak. Danbury's district manager notified the denbury control center (dcc) of the leak at 09:47 and requested an emergency phone conference. The dcc did not receive an alarm or identify any loss of pressure or product from the location of the leak due to the small size of the leak. The dcc began closing upstream remote-control valves at 09:59. Downstream remote-control valves were left open to allow the downstream pump station to draw down line pressure. Denbury began conducting notifications to emergency response officials starting with louisiana state police (lasp) at 10:06, ascension parish emergency operations center (eoc) at 10:18, and iberville parish eoc at 10:22. The national response center (nrc) was notified at 10:33 (nrc #1348131). Denbury (non-contractor) personnel began arriving onsite at 11:50 and, upon verification that the location was safe, began closing the manual 24-inch isolation valve for the receiver at 12:13, which resulted in an immediate reduction in release volume. The valve was verified closed at 12:17 which isolated the receiver barrel. At 14:42 denbury began equalizing differential pressure across the system and at 16:35 on 9/24/2022 the system was returned to normal operation. On 9/28/2022 the receiver trap door o-ring was replaced, and the receiver was pressurized without incident.
