Narrative
On 9/23/2022 at 11:30 am cst, enterprise operator during station rounds discovered leaks below valves in manifolds 100 and 300 at houston ship channel. It was determined that an enterprise operator improperly closed the thermal relief valve 48 hours prior to the leak discovery. This allowed thermal pressure to build up resulting in a leak; all diesel was cleaned up. To prevent recurrence, the thermal relief valve was car sealed open. Operator had oqs suspended until retraining was provided.
