On 9/14/2022, around 1820 the operator tried to start blend a1-270 to shell but one of the pumps (82-p-21a) did not start and both a and b pumps were needed for the blend. The operator checked the breaker at substation 5 but it didn't have any alarms, but reset the pump anyways. At 1857 the blend was attempted again and both pumps (82-p-21 a&b) started normally. The operator noticed a high differential pressure on the pressure indication (82-phs-851c) and proceeded to open up the second set of filters (82phs851a). When the operator went to the field to verify that the valves opened, he noticed that the filter housing had broken off and gasoline was coming out. The blend was immediately shutdown to stop the leak. The blend was restarted with (82-psh-851c) secondary filters online, and failed filters (82psh851a) closed, with no differential pressure issues. 0.43 barrels of gasoline leaked to soil and concrete, all contained within the refinery. A work request was put in to replace the filter/housing and to complete soil remediation. Soil remediation replaced 3 cubic feet of soil and was completed 9/15/22. Shift supervisor, shift superintendent, area superintendent and environmental were notified.
