Narrative
On 9/16/22 at 09:57, a 2nd party row mowing contractor struck the 12-inch corsicana to tyler pipeline with a brush hog at a designed above grade ditch crossing. At 09:58, the control room responded by shutting down the upstream pump and field personnel blocked in the line with mainline valves upstream at corsicana pump station and downstream at tyler meter station. Approximately 200 barrels of crude oil was released and a nearby stream was impacted. Company personnel and oil spill response contractors were mobilized to contain and recover released product. Txrrc was notified and nrc was notified. On 9/17/22 at 18:54, repair work was completed, and the pipeline was restarted. Supplemental report: part g updated to other outside force damage - if damage by car, truck, or other motorized vehicle/equipment not engaged in excavation. The type of vehicle involved in the accident was a john deere 5075e tractor with a 7 ft. Mower. Since type of vehicle cannot be left blank for question 8, motorcycle/atv was selected as the best option. 5mph, motorcycle/atv, off road, 0 and "no" selected for questions 7 through 11 respectively at the recommendation of PHMSA aid to more clearly explain accidents of this nature caused by row mowing activities.
