Narrative
On september 7, 2022, at 16:20, magellan operations personnel were taking a change when product was discovered leaking from an open 2" ball valve at the sump drain pot. The incoming pipeline was shutdown, the ball valve closed, and cleanup activities were initiated. It was determined that the 2" ball valve on the drain pot was mistakenly left in the open position. To prevent a recurrence, the valve will be replaced with a spring-operated valve that must be actively held in the open position. Additionally, procedure 9.01-adm-029 catch and send a mainline change was reviewed with the affected employee, with emphasis on proper valve alignment. After cleanup activities were completed, the line was placed back in service and normal operations resumed.
