At approximately 10:20am, harvest senior operations foreman discovers approximately 20 gallons of crude released from pinhole leak coming from the drain line on three rivers pipeline receiver barrel. Harvest senior operations foreman closes valves and isolates the leak. Harvest senior operations foreman made notifications to harvest foremen. Harvest foreman notified harvest lead operator. At approximately 10:25am, harvest lead operator arrives on scene to evaluate severity, potential impact, safety concerns, and begins cleanup process. At approximately 10:37am, harvest lead operator made notification to ehs specialist. Ehs specialist made notifications to harvest operations manager, harvest control room, and harvest regulatory specialist. At approximately 10:54am, harvest operations team prepares external agency notification. At approximately 11:30am, harvest operations personnel removed and has stored the affected piping. Harvest operations replaced 4" valve upstream of pinhole, installed blind flange on valve, and placed drain back in service. Preventative measure to minimize reoccurrence ' the three rivers pipeline drain piping was redesigned. The excess drain piping was removed and the upstream 4' valve was replaced. Other drain piping on three rivers pipeline system will be evaluated for a similar design. Any similar drain designs will be monitored for internal corrosion.

Narrative

Harvest senior operations foreman closes valves and isolates the leak. Harvest senior operations foreman made notifications to harvest foremen. Harvest foreman notified harvest lead operator. At approximately 10:25am, harvest lead operator arrives on scene to evaluate severity, potential impact, safety concerns, and begins cleanup process. At approximately 10:37am, harvest lead operator made notification to ehs specialist. Ehs specialist made notifications to harvest operations manager, harvest control room, and harvest regulatory specialist. At approximately 10:54am, harvest operations team prepares external agency notification. At approximately 11:30am, harvest operations personnel removed and has stored the affected piping. Harvest operations replaced 4" valve upstream of pinhole, installed blind flange on valve, and placed drain back in service. Preventative measure to minimize reoccurrence ' the three rivers pipeline drain piping was redesigned. The excess drain piping was removed and the upstream 4' valve was replaced.
