Results of the third party analysis indicate trap closure door o-ring seal failure due to rapid gas decompression . 08-23-22 - 10:30 - third party mowers in the area contact fhr operations to report a small ice ball at an fhr trap site. 08-23-22 - 12:28 - fhr operations personnel arrive on site and observe an ice ball formation on the trap door. 08-23-22 - 12:35 - fhr operations personnel verified the trap was isolated from the pipeline. 08-23-22 - 13:25 - trrc notification is made; incident report number 2691. 08-23-22 - 13:41 - pipeline isolated to prepare for replacement of the kicker valve on the trap. 08-23-22 - 16:00 - field repairs begin. 08-23-22 - 18:39 - trap door was replaced with blind flange and kicker valve on trap was also replaced. 08-23-22 - 20:30 - pipeline is placed back in service.
