On august 25, 2022 at approximately 11:14 a release of natural gas liquids (ngl) occurred at the gateway pump station 1 from the meter skid filter that feeds the s505 tx ngl w tx gateway pegasus to jackson, located 17 miles west of midkiff, tx. Measurement techs were on site doing a point-to-point check on the meter station. When valve mov-0201 moved to the open position, the ngl hit the filters causing an abrupt pressure change on the filter vessel door which then caused the door to release and damage the filter vessel and it's associated piping. This caused the release of 1600 barrels of ngl before the meter could be isolated. The filter vessel was never commissioned and used thus the reason that the door was not locked and pinned properly. The filter vessel was removed and on september 1, 2022 at approximately 13:30, the meter station was put back into service. To prevent this from occurring in the future, operations will blind flange any vessels that has not been put into service and use proper lock out/tag out practices and procedures.
