Narrative
On 8/17/22, at 7:05 am, contractors notified enterprise of a leak inside the channel meter station. At 07:15 am, enterprise personnel arrived on site and confirmed the leak was on the channel 8" line. At 08:05 am, the emergency response team arrived with the required personal protective equipment needed to enter the site. At 08:15 am, the leak source was identified to be a pressure gauge on piping installed for the ili assessment. A valve upstream was closed, ending the release. The pressure gauge was removed and replaced with a plug. At 07:50 am, enterprise determined the leak volume exceeded 5 gallons and notified the trrc at 08:29 am, incident report number 2685. Nrc reporting criteria was not met. Investigation of the failure concluded that the pressure gauge had a lower pressure scale range than the pipeline's operating pressure. The pressure gauge failed upon exceedance of its top range of 600 psig. The contractor did not conduct a drug or alcohol test as the pressure gauge was installed 38 hours prior to the accident.
