At 16:47 on august 1, 2022, magellan operations personnel arrived at paola station to perform a line locate when product was discovered leaking from the unit #6 control valve. The valve was isolated and cleanup activities were initiated. The control valve was removed and blind flanges were installed. The #5-12" paola - kansas city line was then placed back into service, bypassing the unit at paola station. The failure analysis determined the upper and lower shaft bushings were both worn, allowing for shaft misalignment, which in turn allowed for excessive wear on the follower shaft seal on one side. The source of the release was the follower shaft seal on the outboard side (non-drive end) of the control valve. To prevent reoccurrence, the valve was replaced with a new, modern control valve. Additionally, a lessons-learned bulletin was created and shared within the company which illustrates the signs of shaft misalignment for field personnel to recognize and advise on the potential need to repair or replace the control valve beyond replacing the follower shaft seal. 2/13/2023 supplemental parts g, h
