On 7/31/22 at approximately 07:45, during a tank switch involving a crude oil receipt local operator opened wrong valve causing a pressure spike (<110% of mop) at the texoma manifold, a-bank causing oil leakage from a bolted flange gasket. The pressure was relieved and controlled and the flange was checked and retorqued by a mechanic. The line was static pressure tested and checked for leaks. Subsequent to the successful static check the line was put back into service. A vacuum truck collected the oil/water mixture and disposed of it in the water recycling facility.
