Narrative
On 7/18/22 at approximately 08:30, operations was notified of a release at smt gathering inlet manifold. All lines going into smt tanks were shut in and investigation began to determine the source of the release. At 16:36, the leak was located on tank 2551 line between the delaware basin header and brooke header resulting in a release of 4.86 barrels of crude oil. Cleanup contractors were mobilized to recover all free product and begin remediation of contaminated soil. All contaminated soil was remediated onsite. Upon visual examination it was determined that the release was caused by an internal corrosion pinhole. A 16-inch plidco clamp was installed to the failure point. Tank 2551 will remain locked out until further decision on repairs can be determined. Supplemental/final update - the tank 2551 line that experienced the failure was receiving some indirect corrosion inhibitor from the product streams on inbound pipelines to the midland terminal but not in sufficient quantity and frequency to prevent internal corrosion growth due to infrequent flow on this tank line. Tank 2551 line was replaced with new piping and is now being directly batch treated with corrosion inhibitor to prevent internal corrosion growth and prevent recurrence.
