Narrative
On wednesday, july 13, 2022, at approximately 08:34, the medallion control room received several alarms from the oems station after a portion of the flow into the station from an adjacent facility operated by another pipeline operator suddenly stopped. When the flow stopped, it caused a vibration in the system and triggered the non-critical alarms. The control room operators immediately acknowledged the alarms, viewed the closed-circuit surveillance camera at the facility and found what appeared to be an oil leak near pump #26520. The controllers then shut down the oems station and dispatched medallion field operations personnel to the site to investigate. The investigation indicates that the flange connection on the discharge control valve off of pump #26520 was leaking. Medallion operations installed a new gasket, torqued the bolts to specification, and returned the station back to normal operations at 11:03 am. Remediation of the affected soil was completed on tuesday, august 9, 2022.
